BACKUP SERVICE FORM

Service Agreement

IMPORTANT-READ CAREFULLY: Do not use the service until you have
carefully read this Agreement, which provides the terms and
conditions for its service. Use of this service indicates that
you have read, understand and accept this Agreement.

1. This service by CAVS provide a backup copy of a JB-99 hard
drive sent in by an owner of JB-99 machine. The format of the
target hard drive into which the data will be copied onto may be
either of PC DOS (compatible to Windows 9x or ME) or Dr-DOS
(compatible to JB-99) chosen by the owner. 

2. CAVS does not take any responsibility or knowledge of the
contents of the hard drives that are sent in from the owner
or copied onto for the backup during the service. The owner is
solely responsible for the contents of the hard drives that are
backup from and to during this service.

3. CAVS does not take any responsibility of a damage or loss of
the hard drive which may occur during a transit from the owner
to CAVS. It is the owner's responsibility to package the hard
drive and select a proper method of transport for the
protection and safe arrival of the hard drive.

4. For any damage or loss of the hard drives due to faulty
handling by CAVS, CAVS shall be responsible and reimburse the
owner by means of exchange, repair or refund. The total amount
of reimbursement however shall not exceed the replacement cost
of the hard drive (with one of equal or greater capacity) plus
the amount paid by the owner for the backup service.

5. In case the customer sends in his/her own hard drive
to be the target hard drive in the backup service along with
his JB-99 hard drive, CAVS shall reformat the target drive and
erase all data contained before the service is performed.

6. Customer of the bacckup service acknowleges that the song
files that are purchased from CAVS or other companies under
licenses can not be played from a target drive when copied. The
licensed song files can only be played when restored in the
original JB-99 hard drive.
